MADISON (WKOW) -- The Madison Metropolitan School District is reminding parents about important health information for kids headed back to school.
Within 30 days of admission to school in the fall, all new students must present immunization documents showing they have received at least the first dose of each type of vaccine required for their grade level. Immunization requirements and online submission forms can be found at the MMSD website.
Students in grades K-8 who have not received the required vaccinations will not be allowed to come to school after Wednesday, October 26. Waivers are available for health, religious, or personal reasons, but parents should be aware that in the event of any vaccine-preventable disease outbreak, students who are not fully immunized may be excluded from school to prevent further spread of the disease.
If a child needs any prescription or over-the-counter medication during the school day, parents and health care providers must fill out specific forms before the medication can be given. Before sending your child to school with medication, contact the school office or nurse for more information.
Parents should also provide the school office with their current home and work phone numbers, plus names and phone numbers for other emergency contacts.
